サービス管理責任者として、

就労移行支援事業所と

中高生向けの就労準備型放課後等デイサービスの

運営に携わり、

利用者さまが将来に向け自信を持って

歩みを進めるためのお手伝いをお願いします。

具体的には、

・利用開始時アセスメント作成

・個別支援計画作成

・モニタリング

・関係者会議への参加

・希望者との面談 など
