【業務内容】
介護老人保健施設にて、入所者さまの
身体状態・生活状況・心理面・家族意向 を整理し、
多職種が 同じ基準で支援できる"共通認識" をつくる役割を担います。
基本的な運動・歩行・姿勢訓練に加え、
専門用語をわかりやすい 生活ベース表現に翻訳
「今できること」「今必要な補助」「将来の想定」を整理
スタッフが共有しやすい 短い支援ガイドライン作成
家族・介護・看護・ケアマネの 意向すり合わせ支援
など、支援のズレを無くし、チーム連携効果を最大化 する業務が中心です。
【1日のスケジュール例】
8:45 出勤・申し送り(情報整理ポイント確認)
9:15 午前リハ(訓練+生活行動データ収集)
11:30 介護スタッフへ"伝えるべきポイント"短報共有
12:00 休憩
13:30 午後リハ(支援方針に合わせた動作訓練)
15:00 多職種ミニ共有会 or 家族向け説明
16:00 記録・"理解しやすい支援コメント"作成
17:30 退勤

【月給】22万円-27万円-≪月給内訳≫ ・基本給:16万円-19万円-・職務手当:2万円-4万円-・処遇改善手当:4,000円-【賞与】年2回 計4.1ヶ月分(前年度実績)
