募集職種: サービス提供責任者

仕事内容: 訪問介護事業所でのサービス提供責任者業務全般

○お客様へのサービス提供

○居宅ケアマネージャーとの連絡調整

○お客様との契約業務

○訪問介護計画書の作成及び各種書類の作成

○訪問スケジュール管理、調整、ヘルパーへの対応

○サービス実施記録の確認

○担当者会議への出席

社用車(AT)または私有車の運転業務あり私用車使用時は1kmあたり25円の借上げで実費支給

業務内容の変更予定なし

資格: ・年齢制限あり(64歳以下)

※定年年齢を上限とした募集(省令1号)

・学歴不問

・介護福祉士必須

・介護職員実務者研修修了者必須

※いずれかの免許・資格所持で可

・普通自動車運転免許必須(AT限定不可)

勤務時間: ・変形労働時間制(1か月単位)

・(1)7時00分~16時00分

・(2)8時30分~17時30分

・(3)9時00分~18時00分

・休憩:60分

※シフトによる(勤務時間は相談に応じます)

・残業:月平均5時間

休日・休暇: ・勤務表による 土、日、祝祭日の勤務もあり(相談応じます)

◆お任せしたいこと

・利用者様・ご家族との面談、アセスメント

・訪問介護計画書の作成、モニタリング

・ケアマネージャーとの連携、サービス担当者会議への出席

・ヘルパーの育成、指導、スケジュール管理

・訪問介護業務

◆サポート体制

・サ責未経験の方も先輩が丁寧に指導

・定期的な研修、勉強会でスキルアップ

・相談しやすいチーム体制

・訪問管理システムを導入しています。(スマホで管理)

給与の備考: ・基本給:170000円~185000円

・処遇改善手当:26,000円~33,000円

※資格手当:0~3,000円

・賞与あり 年3回 賞与月数 計 3.3ヶ月分(前年度実績)

妙高はねうまライン 高田駅から徒歩で4分

営業時間: 事業所の営業時間 月〜日、祝 7:00〜19:00

サービス提供可能時間 月〜日、祝 7:00〜21:00

スタッフ構成: 訪問介護員 常勤(専従)3名 非常勤(専従)3名
