仕事内容:

移動支援専門員のお仕事障がいのある方が、社会参加(余暇活動・外出・通院・買い物など)を目的に外出する際に、

介助者が付き添って安全に移動できるよう支援するサービスです。

障害者総合支援法に基づく「地域生活支援事業」のひとつです。

・外出時の付き添い(公共交通機関の乗り降り、歩行時の見守り・サポート)

・行き先での介助(買い物時の支払い補助、病院での受付など)

・安全面の配慮(混雑した場所での誘導、段差や危険個所のサポート)

・レクリエーション参加の補助(映画館・公園など、本人が望む活動への同行)

※今回募集する移動支援のお仕事は身体介助があります。

(1)外出前の準備

◆ご利用者やその家族と相談し、外出の目的、行き先、時間などを決め、

具体的な支援計画を立てます。

◆電車やバスの時刻、乗り換え方法、バリアフリー状況

(エレベーターやスロープの有無など)を事前に調べます。

◆利用者の体調に合わせて必要な薬や持ち物を確認します。

(2)外出中の付き添い・介助

◆歩行の付き添いや車いすの操作、

公共交通機関の乗り降りを安全にサポートします。

◆目的地での活動支援

*買い物

商品選びの手伝い、レジでの支払い支援。

*公的手続き

市役所や銀行での書類記入の補助、窓口での手続き支援。

*余暇活動

映画館での座席への誘導、図書館での本の検索、イベント会場での案内など。

◆コミュニケーション支援

ご利用者が自分の意思を伝えにくい場合、代弁したり、

コミュニケーションを円滑にする手助けをしたりします。

◆安全確保と体調管理

外出中の転倒防止や交通事故の予防に努め、

利用者の顔色や様子を常に確認し、体調の変化に気を配ります。

(3)外出後の対応

◆利用者を安全に自宅まで送り届け、室内の安全を確認します。

◆その日の支援内容、利用者の様子、発生した出来事などを記録し、

事業所へ報告します。

【身体介助があります】

今回募集する移動支援のお仕事は

国が指定する障害支援区分1以上の方を対象とした

身体介助を伴う業務です。

余暇活動による外出のほか、通院等もあり

移動中の排泄介助、食事介助、服薬介助、乗降介助などが含まれます。

【有資格者の募集となります】

初任者研修修了(旧ホームヘルパー2級)以上の

資格をお持ちの方が対象となります。

ブランクがある方でも問題ありません。
