【主なお仕事について】

・ケアプラン作成・管理業務

・利用者様・ご家族様への相談対応

・サービス事業者との連絡調整

・要介護認定申請代行

・モニタリング・アセスメント実施

・医療専門職との連携業務

利用者様が住み慣れた地域で安心して生活を続けられるよう、多職種協働でケアマネジメントを実践します。日常業務の一部を本社がサポートするため、社員一人ひとりの負担を軽減した働きやすい環境です。
