仕事内容:

利用者様の在宅生活を支える作業療法士として、充実した環境で働きませんか

<具体的な業務>

・訪問看護ステーションでの看護業務に替わるリハビリテーション

・利用者様の日常生活動作(ADL)訓練、生活場面での実践的なリハビリ支援

・書類作成業務(訪問記録、計画書等)

・社内外連絡業務(医療機関、ケアマネージャー等との連携)

・担当者会議への参加

・スタッフミーティング、検討会への参加
