【業務内容】
介護老人保健施設にて、入所者さまが家庭へ戻った際に
"自分の力で続けられる生活動作" を身につけられるよう、実生活に即したリハビリを担当します。
歩行・立位・姿勢などの基本訓練に加え、
生活時間帯に合わせた動作練習
家庭環境を意識した段階トレーニング
一人でできる範囲を広げるサポート
家族との役割再獲得を見据えたプログラム
など、「現場で使える動き」へ落とし込む力 を重視したアプローチです。
医師・看護・介護・ケアマネと連携し
退所後の生活継続ラインを明確化する リハスタイルです。
【1日のスケジュール例】
8:45 出勤・申し送り(家庭復帰プラン共有)
9:15 午前リハ(生活動作中心の実践訓練)
11:30 記録・スタッフ連携
12:00 休憩
13:30 午後リハ(段階トレーニング・セルフケア)
15:00 家族助言・復帰支援ポイントの確認
16:00 記録・翌日の生活課題設定
17:30 退勤
