仕事内容:

*重度訪問外介護でのサービス提供責任者*

Dot365ではご利用者様の障害の特性や生活環境に応じて、

必要なサポートを行います。

その他、管理業務もお任せします

~具体的な業務内容~

*身体介護、重度訪問介護、家事援助、移動支援など*

実際に訪問し、サービスを提供

*サービス計画の作成・調整*

アセスメント(利用者の状況把握)や、訪問介護計画の作成

*スタッフの管理・教育*

シフト作成や、業務指導・研修など

* ご利用者様・ご家族との連携*

定期的に面談し、ご希望のサービスに応えられているか等、

確認を行い、必要に応じてサービス内容を見直す

*関係機関との連携*

ケアマネや医療機関、リハビリ施設などと連携をとり、必要な調整を行う

勤務時間・曜日:

【日勤】

〇7:00~22:00の間の8時間程度(時短勤務は、5時間40分~可能)

※日勤のみの勤務相談可

【夜勤】

〇20:00~翌8:00

固定残業代あり:月給 ¥352,000 〜 ¥420,000は1か月当たりの固定残業代¥65,000(30時間相当分)を含む。30時間を超える残業代は追加で支給する。
