~具体的な業務内容~
* 作業サポート*
ご利用者さんが行う軽作業やパソコン作業などのプログラムを実施。
*心のケア・日常生活のサポート*
精神的な不安を抱えていたり、体調が不安定な方もいるため、
面談と通して、安心して働ける環境を提供します。
*個別支援計画*
支援計画や日々の記録を作成します。
日々の様子を日報に記録し、変化を把握することで最適なサポートに繋がります。
*関係機関との連携*
ご利用者様のご家族との情報共有や、
福祉施設・行政などと連絡を取り合い、必要に応じて打ち合わせを実施します。

勤務時間・曜日:
9:00~18:00の間でシフト制( 勤務時間についてはご相談ください◎)
※休憩は法定通り(6時間以内の勤務の場合0分、6時間以上勤務の場合45分、8時間以上勤務の場合60分)
