【業務内容】
ご利用者さまのご自宅へ訪問し、
「その家にある環境・物・生活習慣を最大活用してできることを増やす」
ことを目的にした "生活資源活用型リハビリ" を担当します。
基本訓練(歩行・姿勢・可動域など)に加え、
家具・家電・配置・手すり位置など 家庭内資源の可能性分析
必要以上に何かを増やさず "今ある環境でできる工夫" の提案
無理なく続けられる"生活に馴染む練習"の設計
生活の質を上げる 省エネ動作&安全性アップ
など、住環境 × 体の使い方 × 継続性 を組み合わせた支援を行います。
ご本人・ご家族・ケアマネとの情報共有を行い、
"特別な設備に頼りすぎない" 生活力の育成 を目指します。
【1日のスケジュール例】
8:45 出勤・訪問準備・生活状況確認
9:00 午前訪問(家庭内資源ヒアリング & 訓練)
12:00 休憩
13:30 午後訪問(動作改善 & 環境活用提案)
16:30 家族・ケアマネ共有
17:30 退勤
