仕事内容:
地域連携課において、部署の管理業務および医療ソーシャルワーカー業務を担当していただきます。
経験・能力に応じて、地域連携課管理者または管理者候補として採用します。
<地域連携課の管理業務>
- 地域連携課の運営全般(業務のとりまとめ、職員の指導・相談対応、勤務調整 等)
- 紹介患者さまの受け入れ方針やベッドコントロールに関する院内調整
- 急性期病院・診療所・介護事業所等との関係構築、連携強化(訪問、連携会議 等)
- 地域連携に関する統計・資料作成、院内会議への報告 など
- ソーシャルワーカー業務(入院・転院・在宅復帰に関する相談援助業務)
*管理業務とソーシャルワーカー業務を兼務していただきます。
*地域連携室や医療ソーシャルワーカーとしてのご経験がある方は歓迎します。
経験に応じて、段階的に管理者としての役割をお任せする場合があります。

社会福祉士または看護師の資格を有する方
医療機関での勤務経験 必須
地域連携もしくは入退院調整に関する業務経験 必須
勤務時間・曜日:
9:00~17:00(休憩1時間)

完全週休2日制(原則、土日祝日)
年間休日125日
年次有給休暇(6か月後10日付与)
慶弔休暇、産前産後休暇、育児休業、介護休業
