【仕事内容】

病院の地域医療連携室にて、医療ソーシャルワーカーとして入退院支援や地域連携業務を担当していただきます。

<主な業務>

・入院、退院、転院に関する相談対応

・患者さん、ご家族からの相談支援

・在宅医療に向けたケアマネジャーとの連絡調整

・地域の病院、開業医との連携業務

・地域医療連携室内での調整業務

・簡単なPC入力、記録作成

【1日の流れ】

・08:45 出勤、連携室内での情報共有

・09:00 入退院相談、調整業務

・11:30 地域医療機関との連絡

・12:30 休憩

・13:30 面談、記録作成

・17:15 業務終了

【入社後の流れ】

入職後は病院や地域医療連携の流れを確認しながら、業務を引き継ぎます。

これまでのご経験を活かしつつ、無理のない範囲から担当業務をお任せします。

【必須条件】

・社会福祉士資格をお持ちの方

・病院での医療ソーシャルワーカー経験1年以上

・Word、Excelの基本入力ができる方

・チーム医療を大切にできる方

給与詳細


基本給:月給 22万8500円 〜 31万5500円

固定残業代:なし

全員に一律で支払われる通勤・皆勤・家族手当金額:なし

全員に一律で支払われるその他手当金額:なし

【月給】228,500円‐315,500円

※面接後決定、経験前職等考慮あり

基本給 153,000円-240,000円

職種加算手当 22,000円

資格手当 45,000円
