月給280,000円~

※介護処遇改善金35,000円含む

月給に以下の手当が別途付与されます

  • 夜勤手当(6000円/回)+時間帯(深夜・早朝)手当
  • 食事手当(250円/日)
  • 資格手当(主任ケアマネジャー:月10000円、介護福祉士:月5000円)
  • 家族手当(配偶者8,400円、第1子・2子 7,000円、第3子・4子4,900円)

<仕事内容>

利用者様の状態やご希望にあわせて、

ケアプランの作成・モニタリング・ご家族との連携などを

行う業務です。

現場の介護業務にも関わりながら、

実情に即した計画を立てていきます。

  • 主な業務内容

・ケアプラン作成(個別援助計画)

・アセスメント・モニタリング

・介護に関する相談対応

・ご家族や関係機関との連絡調整

・一部介護業務あり

登録者数は現在7名。

少人数制だからこそ、一人ひとりにじっくり

向き合える環境です。

職員どうしも連携しやすく、情報共有もスムーズ。

無理なく仕事を進められます。

<働きやすい勤務体制>

  • 着任後は日勤中心で慣れてからシフト調整
  • 夜勤明けの翌日は必ずお休み

時間・勤務日詳細

  • 下記時間でのシフト制

【日勤】8:30~17:30または9:00~18:00

【早出】7:00~16:00

【遅出】13:00~22:00

【ショート夜勤】22:00~翌7:00

・着任後、慣れるまでは日勤中心のシフトです

・実働8時間/休憩60分

・夜勤明けの翌日は必ずお休みとなります

一人ひとりに合わせた支援を、

現場のスタッフと一緒に考えていく環境です。

まずは日勤中心の勤務からスタート。

しっかり慣れてから、無理のないペースで

業務を進めていけます。

福祉の現場で、ちょっとずつ経験を積みたい方にも、

腰を据えて長く働きたい方にも、合っているかもしれません。
