Summary:
As a part of the Goldbelt Glacier Team, you will be supporting medical services for active-duty military personnel, their dependents, eligible DoD civilian employees, and other eligible beneficiaries designated by the Government. As a HealthCare Worker (HCW).
ResponsibilitiesEssential Job Functions:
- Ensure patient care is carried out in accordance with the Standards of Nursing Care and the policies and procedures of the clinic.
- Check patients into the clinic and triage using clinic guidelines. Inform the registered nurse or licensed provider of the patient's condition and potential problems.
- Obtain and document patient and pertinent family history.
- Perform a full range of diagnostic support duties which include taking vital signs, collecting specimens, obtaining, recording and tracking results of diagnostic tools.
- Assist the licensed provider in a variety of diagnostic examinations and procedures such as female cervical screenings (pap smears), skin biopies, and toenail removals, joint injections, long-acting reversible contraception management, by preparing, positioning and monitoring patients, and setting out and handling instruments and equipment, assisting with proper specimen collection, labeling, and transport.
- Perform laboratory tests such as checking urine for sugar and stool for blood. Record and report results of test.
- Perform a range of treatment procedures that include sterile dressing changes, applying compresses, monitoring IV fluids, inserting catheters and suctions, inserting nasogastric tubes, administering medications, giving enemas, setting up and giving treatment that requires auxiliary equipment such as oxygen and suction. Ambulate patients to other areas to include Radiology, various clinics, and physical therapy.
- Prepare patients for tests, examinations, treatments, and/or surgery. Collect specimens such as urine, sputum and stool. Label specimens for laboratory examinations and follow up by using MHS GENESIS / Electronic Health Record or calling the laboratory for reports. Alert provider to conditions which deviate from expected findings.
- Prepare, start, monitor and discontinue intravenous fluids with accuracy and in accordance with established procedures.
- Observe, report and document all observed symptoms, reactions, treatments and changes in the patient condition to the registered nurse or physician. Make careful observations to assess that nursing procedures and treatment do not cause additional distress.
- Perform routine nursing care activities such as taking blood pressures, temperatures, baths and hygiene care, passing and removal of trays, changing of linen, and otherwise assist in the care of the physical needs of the patient.
- Operate basic equipment required in delivery of patient care such as pumps, IV pumps, oxygen administration apparatus and incentive spirometers.
- Execute licensed provider's orders within the guidelines of standard nursing practice. Ensure accurate medication is administered in correct form and dosage to the proper patient as directed by Government supervisor.
- Maintain records of nursing care, dose and time of medication administered, and indicate if the medication was not administered and the reason.
- Administer immunizations in a safe and accurate manner with strict adherence to all Military/Medical and DoD Treatment Facilities and CDC Immunization policies.
- Recognize emergency situations and assist with, or institute emergency measures for sudden adverse developments in patients such as cardiac arrests.
- Perform preoperative procedures for minor surgery, and ensure proper documentation is completed (consent form and procedure checklist).
- Assist patients in admission, transfer, and perform discharge planning follow-up and documentation.
- Support the patient and/or family members toward the achievement of treatment plan goals. Provide instructions to the patient on invasive procedures, surgical procedures and post-surgical conditions which were previously provided to the patient by a nurse or physician.
- Instruct patients on how equipment is used such as oxygen, suction, cardiac monitor and pulse oximeter. Instruct patients and family on use of prescribed medications, contraindications of medications, and the necessity of proper follow-up care.
- Provide emotional support to patients and families.
- Ensure necessary supplies are available and equipment is in functioning order.
- Provide an orderly, clean and safe environment for patients and staff.
- Handle telephone information requests with courtesy, accuracy and respect for patient confidentiality. Receive information and distribute messages as necessary.
- Ensure maintenance and assists with re-supply of pharmaceuticals in the clinic.
- Ensure upkeep and perform checks of emergency equipment i.e., oxygen, emergency cart, suction apparatus, etc. and maintain appropriate logs. Inform the Charge Nurse of any discrepancy on daily checks.
- Perform additional duties within the scope of LPN practice including but not limited to, update of Standards of Nursing Care, and other LPN/LVN duties required for efficient, quality patient care.
- Transport patients to and from other areas of the facility to include hospital clinics and ancillary departments, as ordered by the physician.
- Assist the professional nurse and/or physician with individual, group, and family health instruction in relation to the patient's condition and his/her ability for self-care. Assist in the dissemination of appropriate literature dealing with subjects related to the particular health problems involved. Assist in providing physical and psychological support to patients, assist in the explanation of procedures and treatment, and promote cooperation among staff, patients and families.
- Personnel will utilize MHS Genesis/electronic health record, as required for the performance of their assigned duties. However, use of MHS Genesis shall not be construed as the sole system or platform required. Personnel may also be required to access and utilize other government-furnished systems, software, or platforms necessary for the performance of their duties, as directed by the Government.
- Obtains and records pertinent vital signs, brief history of present complaint, past medical and surgical history, medication reconciliation and indicated preventive/population health services, and provides relevant and individualized age specific care for patients. Documents findings from patient interview in Electronic Health Record / MHS GENESIS. Provide a brief report to the MCMH staff member seeing the patient based on their intake.
- Assesses the patient through their patient interview and reports, to the appropriate team nurse and/or provider, those requiring priority or extensive care in a timely manner.
- Prepares and cares for patients receiving diagnostic tests and specialized treatments according to medical and nursing plans of care.
- Recognizes and institutes emergency measures as needed.
- Assist the Team RN in ensuring efficient MCMH Team workflow.
- Prepares the exam room as needed for office procedures.
- Assists providers and team RN with physical examinations, treatments and diagnostic procedures.
- Assists in maintaining supplies and equipment, per Department SOP. Reports breakage and loss of equipment, safety hazards, and supply deficiencies to the Team RN or appropriate Department staff.
- Provides environmental safety, security, patient privacy, and cleanliness. This includes conducting a routine clean up between patients, and daily/weekly field days.
- Notifies the Team RN and patient if the provider is delayed.
- Ensures reports are available for the provider during prescreening of the health record prior to each scheduled visit
- Assists with clinic check-out and follow-up visits by reviewing post-visit needs with the patient.
- Refer the patient to Medical Clerk for follow-up appointments or specialty care referrals.
- Assist patients by directing or escorting to other clinical services (laboratory, radiology, pharmacy, immunizations, etc.), as required.
- Expected to proactively collaborate and integrate with members of their team, other departments and directorates to achieve optimal care and medical readiness for the patient in the Marine Centered Medical Home model.
- Promotes effective communication with staff, patients, families, and between other departments.
- Instructs patients and/or family members in self-care using clinic resources.
- Prepares patient electronic health record and other appropriate forms prior to patient's appointment
- Utilizes automated programs and information technology tools to facilitate the patient experience.
- Actively participates in MHP Team huddles.
- Assists in the orientation of new personnel as assigned.
- Communicates to patients utilizing asynchronous messaging to improve communication and facilitate care through non-traditional means.
- Administer immunizations to patients as required. HCW must obtain local certification before administering immunizations.
- In outpatient primary care areas, conduct clinical triage, initiate orders, and provide advice and education to patient with common low-acuity symptoms through standardized evidence-based Clinical Support Staff Protocols (CSSP) per the hospital's Standard Operation Procedure
- In ambulatory primary care actively use MHS GENESIS Message Center to communicate with the medical team and patients. Opt into the Message Pools for their clinic and respond to patient messages and coordinate care.
Necessary Skills and Knowledge:
- Maintain a level of productivity comparable with that of other individuals performing similar services.
- Adhere to and comply with the Bureau of Medicine and Surgery, and local instructions and notices in effect during the term of the contract.
- Practice aseptic techniques as necessary; comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection. Function with an awareness and application of safety procedures.
- Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner.
- Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF.
- Maintain an awareness of responsibility and accountability for own professional practice.
- Maintain DoD email account as directed by government supervisor.
- Exercise appropriate delegation of tasks and duties in the direction and coordination of health care team members, patient care, and clinic activities.
- Be able to comply with all applicable Federal, State, and local laws, Department of Defense, Bureau of Medicine and Surgery, and MTF instructions and policies. Proficiency in Microsoft Office software applications including Word, Excel, Access, PowerPoint, and Outlook.
Minimum Qualifications:
- Graduation from an accredited professional nursing educational program.
- Possess a current unrestricted license to practice as an LVN in any one of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.
- Possess a minimum of 1year full-time experience as a Licensed Practical Nurse or Licensed Vocational Nurse caring for adult patients within the past 5 years.
Preferred Qualifications:
- A thorough understanding of the military lifestyle and experience working with military family members.
- Working knowledge of relevant military, state, federal, and local laws and resources.
- Highly developed written, oral, and presentation communication skills.
