仕事内容:

訪問看護ステーションでの正看護師として、ご利用者様のご自宅を訪問し医療と生活の両面から支援を行います。

【主な業務内容】

・ご利用者様宅への訪問看護(1日5~6件程度が標準)

・オンコール対応・緊急出動対応

・訪問記録や書類作成業務

・社内外連絡業務(主治医、ケアマネジャー等との連携)

・担当者会議への参加

・スタッフミーティング、ケースディスカッションへの参加

勤務時間・曜日:

勤務時間: 原則 9:00~18:00(休憩60分)

勤務曜日: 月・火・水・木・金・土・日のシフト制 オンコール当番で出勤する場合があります

補足: ご利用者様の状態によっては土日祝に出勤していただくことがあります(褥瘡処置や点滴対応等、連日訪問しなくてはならない場合)。負担が偏らないようにスタッフ間で分担します。

休暇・休日:

年間休日: 2025年度は合計122日(月10日前後)

有給休暇: 入社当日に10日間付与。入社月によって4月または10月に法定日数を付与します。2時間・半日・1日と選べる有給休暇制度を採用しており、柔軟に取得できます。

リフレッシュ休暇: 有給休暇とは別に年3日付与されます。毎年付与された日から1年間有効となり、有給休暇と組み合わせて取得することも可能です。

その他休暇: 育児休業制度、介護休業制度、慶弔休暇等あり(規定による。育児休業制度取得後、復帰実績あり)

月給:369,100円~431,500円

内訳: 月給(最低保障基本給+定額残業手当)+歩合給+調整給

想定年収: 440~515万円(月70~80コマの場合の歩合給を含む) ※歩合給は40コマ超から支給(1コマ60分訪問)

固定残業代: 時間外労働の有無に関わらず30時間分を固定残業代として支給 69,023円~79,230円

その他手当:

土日祝出勤手当(祝日勤務される方には訪問件数分の割増手当を支給)

年末年始割増手当

通勤手当(上限25,000円/月)※基本、交通公共機関を利用
