【主な業務内容】

・老健入所者および通所者への個別・集団リハビリテーション

・在宅復帰に向けた訪問評価と多職種連携による支援業務

【業務の特徴】

・チームケア:医師や介護職と連携し、包括的なアプローチを実施

・柔軟な働き方:週3日、1日4時間からの勤務が可能
