介護老人保健施設での支援相談員兼介護支援専門員業務

定員100名の介護老人保健施設(空床型ショートステイ含む)において、老健ご利用者の入所時のカンファレンス実施・計画作成を主として、入所調整及び在宅復帰等の退所調整を行う施設介護支援専門員業務。常勤2名の介護支援専門員で業務を分担しています。

相談・栄養科(主任1名(常勤)、支援相談員2名(常勤)、介護支援専門員2名(常勤)、管理栄養士2名(常勤)、相談員補助1名(非常勤)、ドライバー1名(非常勤))に所属し勤務して頂きます。

常勤医師、看護職員、機能訓練指導員、生活相談員、歯科衛生士、管理栄養士などの職員と連携し、在宅復帰支援に力を入れています。

複数の生活相談員(介護支援専門員)が所属しているので、ブランクのある方や経験が浅い方も安心して働くことができます。

経験のある先輩職員が入職後もフォローするので安心です。幅広い年代が活躍しています

通所リハ、居宅、地域包括などが併設しています。

月給 24万4850円~28万4950円

(一律手当を含む)

※表記給料には、一定の前歴換算を含みます。(基準を超える前歴は別途換算)

(資格手当)5,000円(社会福祉士取得の場合)

(宿直手当)5,000円(希望する場合のみ)

・賞与年2 回(基本3ヶ月分)/基本給・資格手当の月給に支給率を乗じて算出

・退職金制度有

・住居手当~30,000 円(家賃が30,000 円未満の場合は家賃相当額)

・扶養手当~26,000 円(配偶者:8,000 円、子ども一人:6,000 円(配偶者がいない場合は12,000 円))

・通勤手当:公共交通機関を利用される場合は、定期代相当額を支給

※バイクを使用される場合は、2km~5km 月額2,000 円、5km~10km 月額4,100円〔10km 超については段階的に増額〕

・年末年始手当〔12/31、1/1〕5,000円/日

交通費:通勤交通費全額支給

※公共交通機関の場合は全額支給

※自転車、バイクの場合は距離に応じて支給(2km~5km 2,100円、5km~10km 4,200円〔10km超については、段階的に増額〕)

※車通勤については、駐車場に限りがありますので、現時点では難しい状況です。

給与例


基本給250,000円、住居手当30,000円(3万円以上の賃貸住宅)、賞与750,000円(夏季250,000円、冬期500,000円)、年収4,110,000円
