【業務内容】

介護施設にて、ご利用者さまの "今の生活の中に眠っている力" を再発見し、

活かせる形で生活へ組み込んでいくリハビリを担当します。

歩行・姿勢・関節可動域などの基本訓練に加え、

できること・できそうなことの丁寧な棚卸し

活用できる身体機能や得意分野の再発掘

本人のペースに合った"生活動作の再構築"

小さな成功を積む声かけとフィードバック

を中心に、「できない動作の克服」ではなく

"できる要素から生活を組み直す" という発想で支援します。

介護・看護スタッフとも共有し、

継続できる生活モデルづくり をチームで行います。

【1日のスケジュール例】

8:45 出勤・情報共有(できた行動の確認)

9:15 午前リハ(行動棚卸し+生活動作トレーニング)

11:30 活動に合わせた小さな助言・姿勢調整

12:00 休憩

13:30 午後リハ(再発見ポイント活用訓練)

15:00 介護スタッフへ活用ポイント共有

16:00 記録・翌日の再発見メモ作成

17:30 退勤
