【業務内容】

有料老人ホームにて、入居者さまが

「自分で選ぶ・決める・望む生活を続けられること」 を目的に、

身体機能だけでなく 意思決定の維持を支えるリハビリ を担当します。

基本訓練(姿勢・歩行・体力維持など)に加え、

本人の希望・こだわり・価値観を踏まえた支援方針検討

「選べる余地」を残す動作・姿勢・生活導線調整

疲れや不安を考慮した無理のない選択肢設計

趣味・日課・社会参加の継続サポート

など、"体を動かすためのリハ" ではなく

"選べる人生を続けるためのリハ" を行います。

介護・看護スタッフと連携しながら、

価値観を尊重するケア計画 の一翼を担います。

【1日のスケジュール例】

8:45 出勤・申し送り(希望・満足度の確認)

9:00 午前リハ(希望に沿った軽負荷訓練・姿勢調整)

11:30 昼食動作フォロー

12:00 休憩

13:30 午後リハ(趣味・日課に必要な動作サポート)

15:00 生活選択肢の維持ポイントをスタッフ共有

16:00 記録・翌日の希望再確認

17:30 退勤
