【業務内容】
ご利用者さまのご自宅を訪問し、日常生活での困りごとを一緒に解決していくリハビリを担当します。
歩行・立ち上がり・姿勢保持などの基本動作訓練だけでなく、
「玄関の段差が怖い」
「家事で疲れやすい」
「トイレまで歩くのが不安」
など、生活の中で実際に起きている課題に寄り添い、環境調整や動作の工夫を提案。
利用者本人や家族としっかりコミュニケーションを取りながら、
"その人らしい暮らし"を長く続けられるよう、無理のないペースで支援します。
ケアマネジャーや看護師との連携も多く、チームで在宅生活を支えていくリハビリです。
【1日のスケジュール例】
8:45 出勤・訪問準備(利用者情報の確認)
9:00 午前訪問(2〜3件/生活動作訓練・環境チェック)
12:00 休憩
13:30 午後訪問(2〜3件/福祉用具アドバイス・家族指導)
16:30 記録・担当者会議・ケアマネ共有
17:30 退勤
